How is it paid for?

Australia: 

Is paid for by a 2% levy on taxable income OVER Au$48,385 pa (£26,365) PLUS from general Taxation. Ambulance cover is by subscription. For a family in South Australia, the premium for one year is Au$165 (£90). The ambulance service is operated by the state.
